What you will need to register for Health Insurance Checklist

Make sure you have all of this information before you start in order to make this process as fast as possible. You will need:

Basic Information about you and your household

Be prepared to answer questions about each member of your household, even if they are not looking for coverage. This information can include the number of people in your family, their relationship to you, full legal names, birthdates, social security numbers, and household address. 

Social Security numbers

Be sure to know the social security numbers of each member of your household, even if they are not looking for coverage.

If you are an immigrant, you will need to have immigration documents

If you are an immigrant, you will likely need to have immigration document information like your permanent resident identification card. This documentation will only be required if your employer needs to verify this information.

Health coverage information

If anyone in your household has a health plan through their employer or through the Federal government, you will need their policy numbers. This will be required with any waiver or if your employer requires to disclose this.

Current doctors and medications

Having a list of your doctors and medications in front of you will be helpful if you have the option to compare and choose from different plans offered by your employer.
